Meeting with your lawyer
It is common for people who have been recently diagnosed with a medical condition, or are expecting to have a medical procedure to document their wishes about future medical treatment and their expectations about quality of life. This is also a way to help your medical decision maker make the right decisions for you if you are unconscious or if having the capacity to make your own decisions is becoming difficult.
We'll catch up so that we can guide you on your options and take your instructions. Then we'll draft the document to make sure your directions are properly recorded and provide it to you for approval and to arrange for signing.
Meeting with your doctor
There are special signing rules when it comes to advance care directives and one of the witnesses to your document must be a doctor (with the other witness a person over 18 years of age). Therefore you'll need to take the document to your doctor to have the document signed. Occasionally your doctor may make suggestions to the content of the document too (in which case we can work with you and your doctor to make the appropriate changes).
Once signed we can store your original advance care directive for you and make you and your decision makers some certified copies in case you require them.
